INNOVATIONS IN CONTINUING MEDICAL EDUCATION
Sharing the Diagnostic Process in the Clinical Teaching Environment: A Case Study
Carlos Cuello-García, MD
A b s t r a c t
Revealing or visualizing the thinking involved in making clinical decisions is a challenge. A case study is presented with a visual implement for sharing the diagnostic process. This technique adapts the Bayesian approach to the case presentation. Pretest probabilities and likelihood ratios are gathered to obtain post-test probabilities of every question considered in a patient. The technique advocates for the judicious use of diagnostic options based on the best evidence available. Clinical decisions, presentation skills, and critical evaluation of the literature are also important elements considered in this method.
Lessons for Practice
- The use of a visual method can help a medical team to share and understand the diagnostic process in a clinical teaching environment
- Medical students and clinicians can boost their confidence in resolving clinical cases by using the Bayesian approach and by implementing the concept of evidence-based medicine
- The clinical sieve, a visual adjunctive, could enhance the teaching process by blending clinical reality and diagnostic reasoning to attain agreement among medical learners, particularly on which diagnostic tests would be helpful
