Retraining Specialists for Primary Care: Exploring Issues at One Academic Medical Center

A b s t r a c t
Issues in developing a retraining program for specialists at an independent
academic medical center were investigated by means of a Delphi survey. Two focus
groups of internal medicine subspecialists assessed their attitudes toward retraining.
The Delphi group identified 17 areas that should be addressed (e.g., Needs Assessment
of Specialists, Curriculum Development - Process and Methodology). The subspecialists
were resistant to the idea of retraining, possibly secondary to their self-perceptions of
having different intellectual needs and approaches to patient care compared with generalists.
These results may be helpful to other institutions anticipating the need to retrain
specialists as part of physician workforce planning. Additional studies will be required
before retraining of specialists may be accepted as a reasonable option in health care
reform.
